Enrollment
Application packets for registration in grades Transitional Kindergarten through Fifth grade are available in the Main Office between the hours of 7:45 a.m. and 4:30 p.m. Applications will be processed once all of the requirements listed below are submitted.
- Completed Enrollment form
- Completed Health History - to be completed by parent/guardian.
- Legal proof of birth. Must bring in an original one of the following:
- Birth Certificate
- Hospital Birth Certificate
- Passport
- Baptismal Certificate
- Picture identification of the parent/guardian who is enrolling the child.
- Verification of address. All documents must be current and must identify the parent or legal guardian. They must show the current date, as well as residential address. Original documents are required. Acceptable forms of address verification include:
- Utility Bills (DWP or Gas)
- Property Taxes
- Rental contract
- Official government mail (CalWorks, Social Security)
- Property deed
- Record of Immunizations. Must be current for the following and indicate the month/date/year administered.
- Students Entering TK & Kindergarten
- Polio - 4 doses*
- DTaP, DPT - 5 doses*
- Hepatitis B - 3 doses
- MMR - 2 doses (Both given on or after 1st birthday)
- Varicella (Chicken pox) - 2 doses
- Students Admitted at Ages 7 & Up
- DTaP, DTP, DT, Tdap, or Td - 4 doses*
- Polio - 4 doses*
- MMR - 2 doses
- Hepatitis B - 3 doses
- Varicella - 2 doses*
- Students Entering TK & Kindergarten
- If your child is enrolling in a public school for the first time, you must turn in the following as well.
- Examination by Physician Card. This is a requirement for first grade entry. Examination may be received with kindergarten enrollment.
- Oral Health Assessment completed by Dentist. (This is optional.)
* Exceptions to the number of doses may vary - inquire with school nurse.
New families who reside within our attendance area may enroll at any time during the school year. Applications must be turned into the school office in person. Be sure to verify school of residence when you come in to pick up an application packet.
LAUSD's first Dual Language Armenian program began at Mountain View Elementary. It is a 50/50 Dual Immersion program. The goal to develop bilingualism and biliteracy with academic content in two languages. 50% of the instructional day is taught in Armenian and 50% in English. Dual Immersion classes are taught by fully credentialed bilingual teachers who have additional preparation and expertise in teaching Armenian. Easter Armenian (Federal orthography) is the primary dialect of instruction.

This program allows parents to apply for designated "open enrollment" seats at specific campuses. The application process typically occurs during May for the following school year. A list of the schools with open enrollment seats is usually available in the main office of any school, beginning in late April or early May.
There is no limit on the number of schools to which a parent/guardian may apply for open enrollment. Schools that still have space available after the deadline usually will be able to register open enrollment students up to the second Friday of the school calendar for that campus.
